Hypertensive retinopathy refers to damage to the retina and its blood vessels caused by high blood pressure.

 


Signs & Symptoms:

 

Most individuals with hypertensive retinopathy do not experience symptoms. However, some may report decreased or blurred vision and headaches.

 


Signs:


Damage to the retina from hypertension may manifest as:


- Arteriolar Changes: This includes general and focal narrowing of arterioles, arteriovenous nicking, changes in the arteriolar wall (arteriosclerosis), and abnormalities where arterioles and venules intersect. Specific signs include Copper wire arterioles, where the central light reflex occupies most of the arteriole’s width, and Silver wire arterioles, where the central light reflex spans the entire width of the arteriole. Arterio-venular (AV) nicking or nipping may also occur due to venous constriction and banking.


- Advanced Retinopathy Lesions: These may include microaneurysms, blot or flame haemorrhages, ischemic changes such as cotton wool spots, hard exudates, and, in severe cases, optic disc swelling (optic disc oedema), a macular star (a ring of exudates around the retina), and visual acuity loss, often due to macular involvement.


- Altered Blood Flow: Strongly modulated blood flow pulse in central and branch arteries can be a result of hypertension. Laser Doppler imaging may reveal changes in hemodynamics non-invasively.

 


Mild signs of hypertensive retinopathy are commonly observed in the general population (3–14% of adults aged ≥40 years), even without hypertension. While hypertensive retinopathy is often considered a diagnostic indicator of a hypertensive emergency, it is not always present.
